Pharmacology/Drugs

  • Normal saline IV bags (1L)
  • Morphine 1mg/mL injectable
  • Hydromorphone 2mg/mL injectable
  • Midazolam 5mg/mL injectable (LU 495 for palliative care)
  • Haloperidol 5mg/mL injectable
  • Doxycycline 100mg oral tablets
  • IM Ceftriaxone with 1% lidocaine without epinephrine for reconstitution
  • Methotrimeprazine 25mg/mL injectable
  • Glycopyrrolate 0.2mg/mL injectable (LU 481 for palliative care)
  • Linezolid 600mg oral tablets (LU 362 when no IV access)
  • Rivaroxaban 15mg oral tablets (LU 444 for treatment of DVT)
  • Each LTC home has an emergency box of medications and is able to report on what drugs are available during a LTC-CARES consultation. The above list is what has been recommended and is typically available, along with oral antibiotics from all major drug classes and other common oral medications.
  • Each LTC home also has access to their emergency pharmacy who should be able to supply additional medications after hours as needed. 

Oxygen Therapy

  • All LTC sites have medical directives to initiate O2 as needed, with a medical order to follow. 
  • Any time O2 is ordered for a resident, a Fax is sent to the LTC site O2 provider (built in process).  An RT provides a home visit within a day or two to assess the resident for home O2 therapy/criteria needs.

Intravenous Access

  • Home and Community Care Support Services Hamilton, Niagara, Haldimand, Brant is available to provide IV insertion service with a turnaround time of a few hours for a new patient. Home and community care support services referrals need to be received by 7pm for same day insertion. Time is needed to open a file, secure a nurse and get needed supplies. A referral to Home and community care support services is required.

LTC Fracture Clinic Splint Access Process

Step LTC patient without LTC-CARES involvement
1LTC team assesses patient and a splint is required. Splint not available at their site or through their procurement partner.
2LTC team places call to Fracture Clinic with splint request; Monday-Friday 0800-1600 (no holidays) @ 905-521-2100 ext. 46989.  Needed information is provided including:  Name, address and phone # of LTC home; DOC contact name and email address, diagnosis, affected body part and side, and splint type required.
3Fracture Clinic completes billing form. The splint is taken to the Hamilton General Hospital Main Entrance/Patient Entrance Screening Desk for taxi pick up. (Hamilton General Hospital, 237 Barton St. East, Hamilton ON, Main Entrance, Patient/Visitor Screening Entrance)
4 Fracture Clinic provides LTC team with pick up time and place.
5 LTC team arranges pick-up and delivery of splint and is responsible for the cost.

Needed information is provided including: Name, address and phone # of LTC home; DOC contact name and email address, diagnosis, affected body part and side, and splint type required.
3Fracture Clinic completes billing process and delivers the splint and bill invoice to the main information desk at 50 Charlton Avenue SJHH site. LTC Facility to coordinate pick up of splint and invoice (via taxi or other services).
4 Fracture Clinic notifies LTC of the delivery time and date.
5 LTC arranges pick-up and delivery of splint and are responsible for the cost of the product.
